About the role

  • Under the general supervision of the Director, Health and Transition Services and in collaboration with the Medical Director, the Nurse Practitioner (NP) works within an interdisciplinary team in related roles as a practitioner, educator and counsellor.
  • The NP improves access to services for people who require health status assessments, periodic monitoring and management, and stable chronic disease surveillance and management.
  • The NP works with individuals, families and groups to promote health and education.

Requirements

  • Proficiency in both official languages (French/English) would be considered an asset.
