Requirements
- Pharm.D. or BS Pharmacy degree from an accredited college of pharmacy Successful completion of Connecticut state pharmacy board examination Active Connecticut Pharmacist License in good standing
- Pharm.D. or BS Pharmacy degree from an accredited college of pharmacy Successful completion of Connecticut state pharmacy board examination
