About the role
- Our innovative Medical Rehabilitation Program offers a meaningful alternative to institutional rehab, helping people heal at home, with dignity and purpose.
- Position Details: Service Area: Stoneham, MA and surrounding areas Job Type: Full-time Schedule: Fri/Sat/Sun or Sat/Sun/Mon - 30 hours per weekend - 3 ten-hour days.
- Patient Care: Assesses and evaluates patient's status by: Writing and initiating plan of care.
Responsibilities
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
- Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care give Provides and maintains a safe environment for the patient.
Requirements
- Preferably 2+ years of nursing experience, with a focus on geriatrics.
