About the role
- As a Hospice Home Health Aide, you will provide compassionate personal care and support to hospice patients in their homes under the supervision of a Registered Nurse.
Responsibilities
- Measure and document vital signs as assigned
- Perform skin care to help prevent pressure injuries
- Maintain a safe and clean patient environment
- Accurately document patient care provided
- Maintain patient confidentiality and uphold HIPAA standards
- Collaborate with the interdisciplinary hospice team to ensure continuity of care

- You will play an essential role in helping patients maintain comfort, dignity, and quality of life while working closely with our clinical and administrative teams to ensure coordinated, high-quality care.
Requirements
- Ability to travel throughout the San Francisco Peninsula (approximately 15-25 mile service radius)
- Travel within the assigned service area is required.
- Current California Home Health Aide (HHA) Certificate
- Minimum 1 year of Home Health Aide or caregiving experience (Hospice experience preferred)
- Current CPR and First Aid Certification
- Valid California Driver's License
- Reliable vehicle with current auto insurance
- Strong communication and interpersonal skills
- Compassionate, dependable, and patient-centered attitude
- Ability to work independently and as part of an interdisciplinary team
- High School Diploma or equivalent
