About the role
- Evaluates medication orders for contraindications; drug-drug interactions; drug-food interactions; allergies and appropriateness of drug, dose, frequency and route.
- Formulate patients' treatment plans and initiate, modify, and continue appropriate drug therapy in accordance with HIV standards of practice or in consultation with the primary physicians.
- Initiate requests for appropriate laboratory and other diagnostic studies as needed for monitoring drug therapy.
Responsibilities
- Provide treatment adherence counseling and support for patients in the HIV Treatment Adherence Program.
- Assists patient and multidisciplinary team with solving medication issues (i.e., prior authorization, patient assistance programs, and financial support programs).
Skills
- Strong understanding of HIV pharmacy services.
- Ability to travel to different clinic sites.
- Demonstrate personal responsibility, respect for self and others, innovation through teamwork.
