As a CADC, you will play a pivotal role in guiding and supporting our patients as they navigate their patient care plans.

Conduct assessments to evaluate patients needs and develop initial patient care plans.
Provide individual and group counseling sessions to support patients in their recovery journey.
Maintain accurate documentation of patient progress and treatment interventions.
Encourage patients to build coping strategies and life skills necessary for lasting recovery.
You'll work closely with our multidisciplinary team to develop personalized patient care plans, facilitate group and individual counseling sessions, and empower patients to achieve their recovery goals.

Collaborate with healthcare professionals and community resources to ensure comprehensive care.
Facilitate support groups and educational workshops for patients and their families.
