About the role

  • Practicing under the supervision of transplant surgeons and physicians, the transplant Nurse Coordinator ensures coordination of the clinical aspects of transplant patient and living donor care throughout the pre-transplant phase.
  • The job duties vary with the particular setting or assignment.

Responsibilities

  • Develops, coordinates, and communicates the patient's plan of care based on ongoing assessment of the patient and input from the patient's family.
  • Engages nursing and support personnel by delegating appropriate tasks per the Illinois State Nurse Practice Act and provides, support and feedback
  • Recognizes and supports Shared Leadership as the selected method of nursing governance.
  • Participates in organizational, departmental and shared leadership activities; supports department and unit goals in daily behavior.

Requirements

  • The Transplant Nurse Coordinator (CNCII) is a Registered Nurse with experience in and knowledge of transplantation and living solid organ donation.
  • Ensures that the patient is added to the UNOS waiting list and the required listing letters are sent in a timely manner per UNOS requirements.

  • Consults with and implements orders by performing interventions as prescribed by the provider and documenting accurately in the electronic medical record.
  • Assesses, engages and obtains clinical information from multi-disciplinary team, patient and/or family to develop an appropriate patient educational plan.
  • Provides cross coverage for the transplant data coordinator by entering living donor information in UNOS prior to the living donor surgery as assigned.
  • Demonstrates a commitment to UI Health's values through teamwork and effective communication in an effort to achieve quality outcomes, patient safety, customer satisfaction and a clean, safe and therapeutic environment.
  • Collaborates with other nurses and physicians to identify, promote and provide positive learning experiences for staff and students.
