About the role

- Revise goals and communicate changes to the physician as needed. - Interdisciplinary Collaboration: Coordinate care within the interdisciplinary team, including goal/discharge planning.
- Provide necessary discharge instructions and home exercise programs to patients and families. - Compliance & Outcomes: Adhere strictly to all regulatory, HIPAA, infection control standards, and company policies.
