As a Federally Qualified Health Center (FQHC), we deliver integrated, whole-person care to individuals and families across Indiana, regardless of income or insurance status.
Educate members on issues related to using the healthcare system and their specific government assistance plan
Maintain HIPAA standards and confidentiality of protected health information
Assist patients with applications and follow-up for Medicaid programs and Marketplace coverage
Participate in outreach and education programs for the community, such as health fairs, screenings, and other events
Assist with Social Determinants of Health where needed
The Community Resource Navigator will serve as a primary point of contact for healthcare patients, as well as the communities we serve, in an ambulatory care setting.
They will support individuals and families entering the healthcare system and assist with utilization and retention of benefits and community resources.
