This is a unique opportunity to provide comprehensive primary and supportive care directly in the homes of some of our most vulnerable members, helping to improve health outcomes and quality of life.
As a Clover Home Care Nurse Practitioner, you'll join our Clover Care Visit (CCV) Team.

You are located in or near Gloucester County, New Jersey and able to travel locally for home visits.
You have an active and unencumbered NJ Registered Nurse license.
You are a Certified Advanced Practice Nurse Practitioner (APN).
You have a master's degree or post-master's certificate in advanced clinical practice.
You have a minimum of 1 year of NP experience in a clinical setting.
You have active DEA and CDS registration.
You have experience in home care or house call settings (preferred, but not required).
