Responsibilities
- Perform preventative screenings and lead important discussions regarding long-term goals of care.
- Review and update patient medication lists to ensure accuracy, and order appropriate laboratory and diagnostic procedures Maintain and review patient records, charts, and other pertinent information accurately and efficiently.
- Maintain a high level of independent clinical decision-making in a fast-paced environment.
- Ensure patients receive care across the entire continuum and provide secondary support within various post-acute settings such as AL/IL and SNFs.
- We invest in our providers' development and support their journey toward excellence.
- This position offers the opportunity to build deep relationships with patients and practice thoughtful, high-quality geriatric care in a supportive environment.
