About the role
- The Care Manager uses independent judgment and discretion to provide direct services including case planning, advocacy, outreach, and home visitation activities for children and families in collaboration with a team.
- Other duties and special projects as assigned.

Requirements
- 2+ years experience providing case management in an intensive level program.
- Has knowledge of and possesses strong engagement skills as well as crisis and de-escalation skills to provide quality care and services to clients and families.
- Possesses a valid driver's license and state-required auto insurance.
- Ability to handle confidential information.
- Establish and maintain effective working relationships with those contacted in the performance of required duties
- Bachelor's degree in Social Work, Health, Public Health, or a related field with relevant work experience
- Background in health care services and/or public health
- Possess the ability to engage and connect with diverse population including but not limited to individuals experiencing homeless, at-risk for homelessness, and under-resourced communities, persons with mental health and substance abuse disabilities
- Comfortable with conducting outreach virtually, in-person, and by phone, including making cold calls
- Detailed oriented - can complete the necessary paperwork (intake forms, Health Action Plan, etc.)
- Available to work evenings or weekends, as needed
- Reliable transportation
Nice to have
- Spanish / English bilingual preferred.
Skills
- Must have intermediate knowledge of computer programs in a Windows environment.
- Database software
- Proficient in Electronic Health Records System
Compensation
- In accordance with California law, the expected pay range for this position is included in this posting.
- The actual compensation will be determined based on factors permitted by law.
- New hires will be offered a salary at the starting point of the range to maintain internal inequity and allow opportunity for future salary growth.
