- The Clinical Pharmacist - Inpatient role is based at Cleveland Clinic South Pointe Hospital where the caregiver works as a decentralized pharmacist primarily supporting the Internal Medicine service line.
Responsibilities
- include participating in multidisciplinary rounds, providing medication management support for hospitalized patients, and contributing to clinical decision-making within the Internal Medicine units.
- Document services in a complete, accurate and timely manner.
- Maintain professional interactions with patients to facilitate successful patient experiences.
- Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for individuals administering immunizations

Requirements
- PharmD or Bachelor's of Science in Pharmacy from an accredited college of Pharmacy
- ability to lift up to 30 lbs.
- Must have ability to clearly communicate verbally by phone or in person
- Follows standard precautions using personal protective equipment as required.
Nice to have
- Board certification
- Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred
- Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area (i.e. Critical Care, Oncology)
