About the role
- The expected salary range for this position is based on several factors including market, funding, candidates experience level, skills, training, and related knowledge.
- The primary function of the practitioner is to provide outreach, housing transition, and short-term housing sustaining services, with the goal of connecting the person to ongoing services.
- Assist in finding resources for household furnishings and supplies for clients who are recently housed or in need.
Responsibilities
- Provide tenancy supports to HUD Section 811 Project Based program participants who are experiencing or who have experienced long term homelessness and serious mental illness in the seven-county metro area.
- Maintain records of progress for each face-to-face meeting and telephone contact with client and collateral contacts.
- Provide post-transition services to assist the program participant in sustaining housing ad living successfully in the community, including making appropriate referrals for long-term support service which may include Targeted Case Management, ARMHS, CADI, ongoing HSS, etc.
- Practitioner will complete all necessary Release of Information and documents for communication with other providers and keep these current for each person served.
- Assist clients to create a plan to work towards service goals in regular meetings with the client, adjusting the Treatment Plan as needed.
- Schedule appointments to see the person to assist in requested services for time and frequency agreed upon in the person's support plan.
- Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
- Drive frequently.
Requirements
- Must be able to transport clients in the community.
- Per agency guidelines all documentation must be completed within 24 hours of a visit.
- Bachelor's degree in behavioral science or related field
- and 2,000 hours of supervised experience in the delivery of clinical services in the treatment of adults with mental illness.
- Bachelor's degree in behavioral sciences or related fields Experience providing outreach services to persons experiencing homelessness and knowledge of services/resources in Ramsey County.

Compensation
- $57,437 + Office
