About the role
- By uniquely aligning the interests of health consumers, providers, and payors, we help to make healthcare accessible and affordable to all populations across the ACA Marketplace, Medicare, and Medicaid.
- We also enable providers to succeed in performance-based arrangements through a suite of technology and services scaled centrally and deployed locally.
- The ideal candidate will be dedicated to delivering comprehensive, patient-centered care, managing both acute and chronic conditions while focusing on preventive health and cost-effective outcomes.
Responsibilities
- Collaborate with care teams, specialists, and ancillary providers to ensure coordinated, integrated care.
- Monitor outcomes and participate in quality improvement initiatives designed to enhance patient care and organizational performance.
Requirements
- Board Certification in Family Medicine or Internal Medicine (required).
- Minimum 3+ years of experience in primary care, with demonstrated Value-Based Care experience.
- A state license to practice medicine in Florida.
- Medical degree (MD or DO) from an accredited institution.
- Strong knowledge of chronic disease management, preventive care, and population health strategies.
- Excellent interpersonal and communication skills with the ability to build long-term patient relationships and collaborate effectively within a multidisciplinary team.
