- We are seeking a certified Texas Emergency Medical Technician-Basic (EMT-B) to serve as a Care Coordinator supporting our Start of Care Operations Team.
- This unique community-based role supports home health operations by conducting drive-by visits in order to locate patients who are unreachable by phone.
- Conduct scheduled and unscheduled in-person drive-bys on potential home health patients who are non-responsive to phone contact attempts by start of care (SOC) operations team.
- Serve as a liaison between the patient and the home health care team to reduce gaps in care and prevent unnecessary hospitalizations
- Perform basic assessments within EMT-B scope of practice to identify any immediate safety concerns or urgent medical needs
- Prior experience in home health, community paramedicine, or EMS
- Reliable personal or agency-provided vehicle (mileage reimbursement available)
