About the role
- Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN)!
- Full-Time, permanent position (40 hours)
- 8:00 AM - 4:30 PM EST, Monday-Friday, with rotating weekends (Saturday or Sunday).
Responsibilities
- Directly support those in hospital patients identified in need of comprehensive case management.
- Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver.
Requirements
- Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
- Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
Nice to have
- Bachelor's Degree in Nursing preferred
- 1 year acute care case management preferred
