About the role
- We are looking for a Team Leader to build an interdisciplinary team, lead with a startup mindset, and create new frameworks for their team to serve vulnerable New Yorkers.
- Join us as Team Leader, Critical Time Intervention (CTI), where you'll oversee the operations and services of a new and dynamic program, ensuring it delivers life-changing results with precision and care.
Responsibilities
- Liaises with partnered hospital systems to manage referrals and facilitate access to services.
- Manages outreach and engagement, admissions and discharges, assessment, case presentations, and team assignments.
- Monitors phases of services and manages caseloads according to phases.
- Maintains and updates resource list for staff.
- Coordinate staff schedules and manage on-call requirements ensuring timely response to referrals and clinical events.
Requirements
- Ensures required reporting to regulatory bodies and third-party payers.
- Submits other reports as required.
- A Licensed Practitioner of the Healing Arts (LPHA) with 3-5 years of post-licensure experience working with individuals who have mental health, or co-occurring mental health and substance use disorders, preferably with experience collaborating with hospital systems.
- Demonstrated competency in clinical documentation, motivational interviewing, stage-wise treatment, and motivational interviewing.
- Strong written and verbal communication skills and strong leadership skills/
Nice to have
- Previous management and supervisory experience preferred.
Compensation
- $97k-$97k

- Responds to inpatient and ER admission alerts and contacts medical providers/hospitals for admission/discharge information.
- Responsible for payroll reports, reviewing employee timesheets, monitoring hours worked and vacation time.
