Responsibilities
- Lead and participate in family meetings and goals-of-care discussions
- Collaborate with interdisciplinary team members including nursing, social work, chaplaincy, and case management
- Support transitions of care, including coordination with hospice when appropriate
Requirements

- Board Certified in Hospice & Palliative Medicine
- Board Certification in Internal Medicine, Family Medicine, or have completed a Geriatrics Fellowship
- Texas medical license (or ability to obtain)
- Strong communication and team collaboration skills
