About the role
- Participates in development of client's service plan, consistent with assessment.
- Arranges for delivery of needed services, as identified in assessment.
- Assists client in accessing needed services through, for example, referrals or linkages.
Responsibilities
- Additionally, all staff are expected to develop a working knowledge of and follow all policies and procedures related to safety management and other Joint Commission standards.
Requirements
- Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or related field as approved by Medicaid, or as required by applicable funding source.
- Minimum of zero year's experience in case management/service coordination with a Bachelor's degree.
- Working knowledge of mental and physical characteristics of assigned population group (e. g., growth and development), human behavior, treatment/service options, and related issues.
- Working knowledge of SCS programs and services, policy, and procedure.
- Working knowledge of applicable regulations and licensure standards.
Skills
- Good interpersonal skills and written and oral communication skills.
