Responsibilities
- Directly supervise a team of Care Coordinators and their day-to-day operations Provide leadership, onboarding, training, coaching and development to team of direct reports Create & foster a positive, fulfilling work environment Provide your Regional team of Care Coordinators with direct support, at regular intervals, and encourage professional growth Adequately communicate job expectations & parameters to direct reports, ensuring quality patient care is always delivered Manage all teammate employment activity: hiring, promoting, job performance, disciplinary actions, termination, etc.

- The ideal candidate will live in the Vacaville or Sacramento area.

Compensation
- The Salary Range for the role is $57,784.00 - $76,000.00 per year.
