Responsibilities
- Provide direct nursing care in homes without assigned staff to maintain continuity and quality of care.
- Identify and report trends in staffing gaps and collaborate with the DON and Care Team Directors to address them.
- Support interim care planning and execution until permanent staffing is secured.
- Collaborate with Patient Coordinators and Directors to align care delivery with patient needs.
- Clinical Oversight & Support
- This role ensures continuity of care, promotes patient engagement, and supports compliance with clinical and regulatory standards.
- In addition to clinical responsibilities, the RN Bridge Nurse collaborates closely with the Operations team - including Care
- While this position reports directly to the DON, the RN Bridge Nurse maintains a collaborative (dotted-line) relationship with the Executive Director to ensure coordination between clinical and operational priorities.
Requirements
- home health or transitional care experience preferred.
- Strong communication and interpersonal skills, with a focus on patient-centered care.
- Knowledge of DOL/EEOICPA guidelines and home health regulations a plus.

Nice to have
- Active Registered Nurse (RN) license in the state of practice (BSN preferred).
- 3+ years of nursing experience
- Proficient in EMR systems
- experience with IGEA preferred.
Skills
- Field-based with travel to patient homes as needed.
- Involves regular coordination and communication with Operations and Clinical leadership.
