About the role
- Job Description: The EPC Psych Case Manager position will receive referrals from both inpatient and outpatient services.
- Clients can be either EPC or voluntary referrals.
- This person will be responsible to p erform an Intake Assessment and Individualized Program Plan for each client.
Responsibilities
- Provide on-going support, advocacy, and crisis management follow-up services for referred patients.
Requirements
- CERTIFICATIONS/LICENSES: Valid NE Drivers License required.
Nice to have
- Preferred 2 years of clinical experience in case management related activities for persons with severe and persistent mental illness.
- Preferred clinical experience with substance abuse treatment issues.
- Valid NE Drivers License required.
- Basic Life Support (BLS) through American Heart Association (AHA) required within 3 months of hire.
- CPI required within 3 months of hire.
