About the role
- You will work with patients of all ages with differing complexity of palliative care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

Responsibilities
- Engaging patients using a collaborative approach by building rapport and trust
- We may use artificial intelligence (AI) tools to support certain stages of the recruitment process, such as reviewing applications, analyzing resumes, or assessing candidate responses.
- Opportunity to develop long term therapeutic relationships with patients
Requirements
- Valid driver's license and access to a vehicle is required
- Experience in delivering community care is an asset
- Experience with children is mandatory for this position.
- Registration with the CNO as a Registered Nurse in good standing
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
