About the role
- Under the supervision of the Program Director the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients.
- The Care Coordinator will coordinate individualized service including orientation, transportation, doctor visits, and schedule.
- Ensures programs and services are implemented at the highest standards and clients receive the highest level of care.
Responsibilities
- Document shift reports and share pertinent information with the team.
- Coordinate the transportation schedule for all clients and drive clients as needed.
- Document all transportation and group activity in EMR.
- Maintain the confidential nature of clients and related activities.
Requirements
- Must have a valid driver's license and eligibility for insurance with agency's vehicles.
- Must have valid First-Aid & CPR Certification or be willing to obtain.
- Minimum of two (2) years driving experience required.
- Must have strong written communication skills.
Nice to have
- The Care Coordinator will have a minimum of a high school diploma, Bachelor's degree preferred.
- Experience with junior high or high school students is preferred.

- Proficient in basic computer skills.
- Engage in appropriate written and verbal communication with families and other professionals.
