About the role
- The NP will also provide consultation and advice regarding immunization of at-risk patients.
- Nurse Practitioners function as autonomous and collaborative healthcare providers, responsible for the assessment, diagnosis, treatment, and ongoing management of patients within the Infectious Diseases, Immunology and Allergy program.
- In addition to clinical responsibilities, the NP participates in program development, quality improvement initiatives, and the evaluation of clinical services.
Responsibilities
- Performs and documents comprehensive patient assessments based on thorough history taking from the patient and/or caregiver and physical examination
- Develops a plan of care in collaboration with the patient, family and interdisciplinary team as appropriate
- Collaborates with interdisciplinary team members to create holistic plans of care for patients
- Performs skills and procedures as per competency within clinical specialty
- Documents all elements of patient work-up and care in the EMR during or following each patient encounter
- Identifies, leads and implements specialty specific program initiatives and improvements for care delivery
- The role supports timely access to care and contributes to improved patient outcomes through clinical expertise, care coordination, and system navigation.
Requirements
- Vulnerable Sector Check (Required) (Essential)
- Master's degree in Nursing or relevant alternative (Essential)
- Experience with program development, implementation and evaluation (Essential)
- Electronic technology proficiency including but not limited to
- Bilingualism (French/English) will be validated by a language proficiency assessment conducted by a third party provider.
- Candidates will be required to meet the minimum linguistic profile prior to the date of hire.

- Current Professional Liability Protection (PLP) (Preferred)
- Bilingual in English and French (Preferred)
