Responsibilities
- Whether it's residential support, respite care, peer support, or employment assistance, our team shows up with compassion, patience, and the drive to make a real difference.
- The QP independently manages and coordinates the overall development, implementation, and monitoring of client-centered habilitative programming, including individualized goals, behavior guidelines, and service plans.
- Develop objectives, oversee data collection, and monitor measurable outcomes.
- Address and document staff performance issues in a timely and professional manner.
- Document communications with families/guardians, Care Managers, and stakeholders as appropriate.
- Develop Plans of Care, task analyses, and training materials; train staff and families as needed.
Requirements
- Coordinate, implement, and monitor Service Plans and Plans of Care within required timelines.
- Review and sign service documentation at least monthly and participate in required monitoring visits.
- Staff Supervision, Training & Coaching Observe and supervise direct care staff at least quarterly or more frequently as required.
