About the role
- We are seeking a highly engaged and patient-focused Care Navigator/Field Support professional to support the In-Patient Case Management team. This role will report directly to the Director and Senior Manager of In-Patient Case Management and will serve as a key liaison between patients, providers, and case managers to help ensure continuity of care, timely
