- The role is responsible for program activities such as referral coordination, eligibility checks, and monitoring CHW caseloads.
Requirements
- High School Diploma required.
- Minimum of 3 years of experience with data collection, management, and/or analysis.
- Minimum of 2 years of experience extracting information and exporting data to other applications.
Nice to have
- Experience with the Epic EMR system is a plus.
Compensation
- $25.00 - $29.32 hourly
