About the role

  • The Case Manager is a Registered Professional Nurse who demonstrates expanded knowledge and experience in providing care for an identified clinical specialty or case type.
  • Responsibilities include, but not limited to: Accountable for desired clinical and cost-effective outcomes, collaborative practice, the allocation of human and material resources needed for care delivery, discharge planning, and evaluation of length of stay.
  • Practices according to the established standards for case management and has attained competencies.

Responsibilities

  • Utilizes the case management process to perform a leading role in improving the quality and efficacy of care provided.
  • Develops integrated and coordinated approach to patient outcomes, and monitors care delivery and interdisciplinary clinical practice patterns to achieve a balance between quality and cost (desired outcomes).

Requirements

  • Will possess the ability to assess data reflective of the patient's status and interpret the appropriate information necessary to provide quality care.

Nice to have

  • Bachelor of Science in Nursing degree-preferred.

Skills

  • Will demonstrate the knowledge and skills necessary in providing case management practices which are appropriate to the age of the patient.
