Responsibilities
- Provide on-call psychosocial support to hospice patients, caregivers, and families by phone and in person.
- Support admissions, changes in level of care, and end-of-life needs in collaboration with the after-hours care team.
- Strong skills in crisis intervention, grief support, case management, and critical thinking.

Nice to have
- LCSW or licensure eligible preferred
- At least three years of social work or counseling experience
- hospice or end-of-life care preferred.
Compensation
- PRN (as-needed) position.
- Team members are required to pick up a minimum of 15-20 shifts per month.
- Available shift options include:
- Weekdays: 5:00 PM - 8:30
