About the role
- The Engagement Specialist serves as a key relationship-builder and community presence within Matter Health's care model.
- This role is responsible for engaging older adults living within assigned communities, helping patients navigate their healthcare journey, and supporting ongoing patient connection, outreach, enrollment, and retention efforts.
- Engagement Specialists work closely with operational and clinical teams to help ensure patients remain engaged, supported, and connected to care.
