About the role

  • Ennoble Care's clinicians go to the home of the patient, providing continuum of care for those with chronic conditions and limited mobility.
  • Ennoble Care offers a variety of programs including, remote patient monitoring, behavioral health management, and chronic care management, to ensure that our patients receive the highest quality of care by a team they know and trust.
  • Purpose: Ennoble Care is hiring for a full-time Hospice RN Case Manager who resides within or in close proximity to Bucks County, PA.

Responsibilities

  • Manage your assigned patient caseload within your territory and be responsible for the following tasks, including but not limited to: assessing, planning, implementing, and evaluating of patients care throughout all phases of the nursing process.
  • Document all notes in the EMR and submit them accurately and in a timely manner
  • Communicate and partner cross-departmentally with all attending physicians, hospice staff members, and other agencies as needed to coordinate optimal care for all patients
  • Partner with clinical leadership to manage/address any issues, share professional support, and exchange feedback to enhance professional growth.
  • Supervise and document the NAC every two weeks or as assigned
  • Provide appropriate support at time of death and period of bereavement for family members and loved ones
  • Maintain familiarity with policies of the agency and rules and regulations of State and Federal bodies which govern hospice
  • Support - from our in house clinical team with any requests

Requirements

  • 1-2+ years of work experience as a professional nurse preferably at least one of the years in hospice care
  • Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.
  • Ability to make meaningful observations, skilled assessments, and write comprehensive, accurate reports
  • Must be a team player who supports an interdisciplinary team concept of caring for patient and loved ones/caregivers
  • Autonomy - so you can feel trusted to provide the best care possible

Compensation

  • Starting at $90,000 salary
