Responsibilities

  • Documents functional, situational, and clinical assessment within 24 hours of patient/client contact.

Requirements

  • 2 years experience working with individuals with HIV or
  • Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

Nice to have

  • Master's Degree Social Work, related Social Sciences, Psychology and
  • 1 year experience working with individuals with HIV. or
  • Licensed Social Worker
  • State of Pennsylvania Upon Hire
  • case management training as indicated by DOH or grantor requirements and
  • LSW - Licensed Social Worker - State of Pennsylvania Upon Hire
