Responsibilities
- Manage chronic and complex geriatric patients in LTC/SNF settings
- Collaborate with facility nursing and administrative teams
- Document care in EMR in a timely manner
- Build strong relationships with rural facilities and care teams
Requirements
- Experience in LTC, SNF, geriatrics, post ‑ acute care, or rural medicine
- Familiarity with Medicare and value ‑ based care
- MD or DO
- Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics
- Active CO medical license (or ability to obtain)
- DEA license
- Interest in leadership or future program growth
