Responsibilities
- Complete evaluations and develop individualized plans of care
- Collaborate with physicians, PTAs, nurses, and the interdisciplinary care team
- Support continuity of care through admissions, transfers, and discharges

Requirements
- Minimum of 1 year of PT experience in an acute care setting required
