- Job Overview The RN Case Manager may provide daily care coordination, case management, coaching, consultation, and intervention to patients with one or more chronic diseases.
Requirements
- Experience with physically or mentally impaired adults and/or the geriatric population.
Nice to have
- Current valid License as a Registered Nurse through the California Board of Registered Nursing
- a minimum of 2 years of managed care experience in case management with a focus on inpatient and/or outpatient ambulatory care is preferred.
- Bilingual in English and Spanish preferred.
Compensation
- Pay for this job starts at $104,000.00 annually
