- Conducts joint home visits with admissions nurses when psychosocial complexity is present (e.g., patients living alone, lacking caregivers, or experiencing mental health/substance use concerns).

Responsibilities
- We are able to support hundreds of individuals in our community each day because of our incredible staff.
- Collaborates with hospice interdisciplinary team, hospitals, and care facilities to develop comfort-focused plans of care, complete comprehensive assessments, and evaluate ongoing interventions.
- Collaborates with patients, families, and case managers during hospital discharge planning to ensure safe and appropriate transitions of care.
- Performs informational and follow up visits for patients in hospitals, facilities, and home settings.
- Supports initial response for urgent referrals needing immediate response.
Requirements
- LSW or LCSW is also required or must be obtained within 2 years of hire date.
- MSW required
- Two years' experience in the health care setting preferred.
- We offer a full range of benefits including:

Compensation
- Full Time - 0.8 or 64 hrs. per biweekly pay period
- Every 5th-6th weekend, 1-2 holidays per year
