About the role

  • The Community Outreach Team is a dedicated group responsible for promoting Foodsmart's foodcare program in partnership with Santa Clara Family Health Plan (SCFHP).
  • We are seeking a candidate who possesses strong interpersonal skills, a deep knowledge of the Santa Clara County community, and a passionate belief in Foodsmart's commitment to food as medicine.
  • Please note that this is a part-time position with up to 25 working hours each week.

Responsibilities

  • 🤝 One Team - You collaborate with respect and commit as a group.
  • Cultivate relationships with identified organizations to build awareness of Foodsmart.
  • Maintain a comprehensive activity log tracking events, referrals generated, appointments scheduled, and enrollment outcomes to share with the internal CHOC team.

Requirements

  • Bilingual in English and Spanish (required).
  • Bilingual fluency in English and Spanish (required).

Compensation

  • $45k-$45k

  • Support the CHC (Community Health Center) referral activation strategy, including outreach to FQHCs and CBOs across Santa Clara County.
  • Willing and able to travel to community events, health centers, and FQHC sites throughout Santa Clara County.
  • Identify and evaluate local community events, health fairs, and clinic-based opportunities for onsite Foodsmart presence in Santa Clara County.
