About the role
- In addition to core coordination responsibilities, this role carries a secondary focus on Crisis Response and Program Engagement, supporting participant safety, program stability, and a positive, inclusive environment.
Responsibilities
- Manage and coordinate services for a caseload of approximately 40-45 participants, ensuring alignment with ISP, BIP (as applicable), and individual goals
- Coordinate support for participants attending day-program services, ensuring consistency and continuity of care across all environments
- Review, manage, and oversee Incident Reports (IRs) for accuracy, compliance, and timely follow-up
- Identify barriers to participation, safety, and goal achievement, and help develop solutions
- Collaborate with Behavior Team and Program Managers to ensure consistent implementation of crisis plans and debrief processes
- Support participant engagement initiatives such as STAR of the Month and other recognition programs
- Contribute to a positive program culture through encouragement, recognition, and morale-building efforts
Requirements
- High school diploma or equivalent required
- Minimum of 2 years of experience in developmental disabilities (DD) services, behavioral support, case management, residential services, or a related human services field
- Familiarity with Colorado's Medicaid Waiver system (HCBS-DD, SLS)
- Valid driver's license and reliable transportation, if travel between program sites is required.
- Must be able to successfully pass a background check and drug screen and obtain DOT Certification.
- Ability to remain calm and effective during high-pressure situations
- Ability to obtain and maintain CPR/First Aid certification
