Collaborates with physician, patient, and team members to establish, implement, and evaluate a plan of care including goals and periodically revises patient's treatment/ care plan

Obtains required physician orders for all care provided.
Bachelor's Degree (B.S.) in occupational therapy or equivalent; Master's Degree preferred

One year's experience in a health care setting preferred
Notifies the patient's attending physician and other appropriate healthcare professionals and staff members of significant changes in the patient's condition in a timely manner
