About the role

  • We assist our patients by coordinating all aspects of their care and by helping our clients/families make informed decisions to obtain the best health care options possible.

  • Positions are based primarily in the 680/24 corridor and surrounding areas and include full-time and part-time (15-30 hours per week) opportunities.

Responsibilities

  • Support and coaching for office visits, including all appropriate documentation

  • Strong organizational skills and ability to prioritize tasks quickly and effectively
  • Ability to be a strong yet tactful advocate for patients
  • Ability to work independently
  • Ability to use iPhone as a professional technology tool

Nice to have

  • Preferred 5 years nursing experience, including acute care, but will consider a strong candidate with less experience
  • RN Case Management and Community RN experience a plus
  • Certification in Case Management (i.e., through CMSA) a plus
