About the role
- The ultimate goal of the program is to transition participants into long-term housing.
- Reports to: Facility Director Full-time.
- Open Until Filled - First review of applications will occur 7/27/2026.
Responsibilities
- Every participant in our reentry program is assigned a case manager to provide support.
- Case managers are also responsible for identifying appropriate resources to support participant success.
- Help participants achieve and maintain program compliance.
- Collaborate with other professionals involved in the participant's success, such as: sponsors, doctors, lawyers, DHS workers, counselors, peer support specialists, family members, parole officers, etc.
- Maintain confidentiality of each person involved in our reentry program.
- Develop relationships with local property managers, landlords, housing authorities, and developers of affordable housing.
- Maintain current information on housing opportunities, community resources and applicable laws.
Requirements
- Occasionally required to move about within the facility for different meetings, office equipment, etc.
- Key Accountabilities: Case Management: Assess each participant's needs and obtain knowledge of each person's history through interviewing, records review, and consultation.
Nice to have
- Direct or Lived Experience with homelessness, addiction, and/or low-income populations.

- Excellent interpersonal, oral, and written communication skills.

- Must be able to work a flexible schedule with the possibility of some occasional early morning, evening, and weekend work. $22.71 per hour, full-time benefit eligible, medical/dental/vision/PTO/401k.
- Build a resource network to support participants in a variety of areas, such as: medical, mental health, DUI classes, social groups, financial counseling/coaching, legal support, etc.
- Provide education and resources to participants on how to secure and maintain permanent housing.
