About the role

  • H2O PTTS Resource Specialist provides housing transition/navigation services and housing pre-tenancy and tenancy-sustaining services.
  • Adhere to the policies and procedures for the H2O Housing Pre-Tenancy/Tenancy Sustaining Services program and the organization.
  • Locate and engage referred participants.

Responsibilities

  • The role is responsible for ongoing needs assessments, personal treatment plans, implementation, support, information, referrals, and advocacy for individuals who are experiencing homelessness or at risk of homelessness.
  • This position requires adaptability to varying routes and schedules, as well as partnering with staff and community partners to provide supportive services to referred participants to increase their self-reliance.
  • Maintain a caseload of no more than 15 participants at a given time.
  • Attend and provide support to participants during voucher briefings and attend match meetings.
  • Assist participants with developing a housing care plan and support for the follow-through and achievement of the goals defined in the plan.
  • Coach participants on developing and maintaining key relationships with landlords/property managers.
  • Attend trainings that increase knowledge and maintain high fidelity to the model.

Requirements

  • Must have held a valid driver's license for the last 5 Years.
  • Demonstrated proficiency with basic computer systems and software, including Microsoft Office products, including Outlook, Word, and Excel, and web-based data systems.
  • First Aid/CPR certification or ability to obtain.
  • Must be able to satisfactorily complete the background check process.
  • Assist participants with obtaining required documents to secure housing (e.g., Social Security Card, birth certificate, income verification, prior rental history).

Nice to have

  • Bilingual (English/Spanish).
  • At least two years of experience working with persons with Serious Mental Illness (SMI).
  • PHYSICAL ENVIRONMENT/CONDITIONS Ability to stand, walk, and work outside in streets, washes, and desert homeless camps (in the desert climate) for up to four hours per shift.
  • Ability to work in an office environment, seated and/or standing, with moderate office noise.
  • Ability to stand, stoop, bend, and lift up to 40 pounds.
  • Visual acuity to discern data and information, and hand dexterity to accurately enter information into computer systems.
