About the role

  • This is a full-time position with IMPaCT Care.
  • You will be a direct employee of IMPaCT Care, working embedded within our Kaiser Permanente partnership in your region.
  • As a Community Health Worker, you'll be a vital link between high-risk patients and the care and services they need - working in the community, not behind a desk.

Requirements

  • TB screening, Hepatitis B, Measles, Mumps, Rubella, Varicella, Tdap/Pertussis, Influenza (required August 1-April 30) Work Authorization Applicants must be authorized to work in the United States.
