About the role
- Expected schedule will be 3x12s (Monday-Wednesday or Wednesday-Friday) 7:00am-7:00pm CST.
- Nurses must reside in Tennessee, Missouri, Louisiana or Illinois.
Responsibilities
- Perform a comprehensive assessment of a patient's clinical, psychosocial, discharge planning and financial needs. Establishes clinical milestones and goals related to these issues.
- Document all care management assessments and interventions.
- Perform other duties as assigned
Requirements
- Bachelors in nursing from an accredited university required.
- 5+ years of pediatrics experience required
Nice to have
- 1+ years' experience in care coordination or case management experience preferred
- B ilingual Spanish preferred
- Familiarity with Medicaid regulations and services a plus
- Value Based Care (VBC) experience a plus
- Virtual care experience a plus
