About the role
- Responsibilities: Job Summary: Case Management Extenders role is to support varies functions of Case Management that can be performed by a non-licensed care team member. They are able to assist the patient and follow the patients individualized plan of care with the guidance of a professional case manager. Provides administrative support to Case Management
- teams as needed to support workflow process and outcomes to include: answer multiple and designated telephone lines, prioritize and route calls, demonstrates effective and interpersonal communication skills, and utilizes computer skills at an intermediate level. Collaborates with Case Manager / Social Worker to provide post-acute referral for discharge
