- This is a hands-on clinical leadership role for an LPN who is passionate about resident care, team development, medication management, regulatory compliance, and creating a warm, supportive environment for seniors.
Responsibilities
- Lead, supervise, coach, and support CNAs, CMAs, medication aides, caregivers, and personal care staff.
- Manage eMAR documentation, medication administration oversight, pharmacy coordination, and medication-related compliance.
- Maintain accurate clinical documentation and prepare for surveys, audits, and state inspections.
Requirements
- Current Licensed Practical Nurse (LPN) license in Virginia required.
- At least 2 years of nursing leadership, supervisory, or management experience required.
- Strong knowledge of resident assessments, care planning, medication management, clinical documentation, and regulatory compliance.

Nice to have

- Spring Oak Senior Living at Warrenton is seeking an experienced Licensed Practical Nurse (LPN ) to serve as the Wellness Director for our assisted living community.
- The Wellness Director reports directly to the Executive Director and works closely with regional and corporate clinical leadership to support quality care, strong communication, and day-to-day clinical operations.
- Oversee resident assessments, service plans, care plans, and wellness services.
- Coordinate care with physicians, hospitals, hospice, home health, rehabilitation providers, pharmacies, and families.
