About the role

  • By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.
  • As a Home Health Physical Therapist, you will: Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
  • Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.

Responsibilities

  • Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
  • Document patient observations, interventions, and evaluations promptly and thoroughly.
  • This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance.

Requirements

  • Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

  • One year of experience as a Physical Therapist Pay Per Visit/Unit Rate $59.00 - $83.00
  • If selected for this role, you will be required to be screened for TB.

  • Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
