About the role
- Complete an initial assessment to identify resources and needs and coordinate the development of an i ndividualized, goal-oriented service plan for each client.
- Participate in an on-call rotation to provide 24-hour individualized care.
- I ncrease job knowledge through involvement with in-service trainings and educational trainings appropriate for the position.
Responsibilities
- Assist in developing formal and informal community supports and network of family, friends, and professionals.
- Participate in weekly staffing supervision meetings with collaboration partners.
- Implement a wrap-around approach to support individuals, family members, and other significant persons in their lives.
Requirements
- Bachelor's degree in psychology, sociology, or related human services field, or two years direct care experience or training in human resources field.
- CPR/First Aid required (will provide training at no cost).
- Proof of valid driver's license and insurance is required.
